New Clients 

I am an in-network participating provider with the following insurance carriers: 

  • Regence 

  • Premera

  • First Choice Health Plans

  • LifeWise

  • Anthem

  • Blue Cross / Blue Shield

Please call your insurance provider prior to our first appointment to verify your coverage and determine your copay or other related fees for psychiatric and mental health services.  Some insurance companies I am in-network with subcontract with smaller insurance carriers with whom I am not in-network, and therefore any services rendered may not be covered. 

I am not a participating provider with Kaiser Permanente, Medicaid, or any managed care plans, including Apple Health, Molina, Coordinated Care, and Ambetter.If you do not have health insurance or I am not a participating provider with your insurance plan, I offer services at a private pay rate.  Private pay rates are available upon request.

 

Scheduling Your First Appointment 

  • Call or email the office using the inquiry form on the Contact Page. Please note:

    • The individual seeking treatment must make contact to schedule an appointment.

    • Email and electronic form submission are not secure methods of communication.  Please be mindful of the information you include via this form.

  • My practice manager, Shawn, will then contact you to gather some initial information and answer any questions you might have. Please allow 48 hours for a response. You also may be asked to provide further information regarding reasons for seeking treatment and relevant psychiatric or medical history in order to determine whether I might be a good fit for you as a treatment provider. 

 

  • Once the two initial evaluation appointments are scheduled, you will receive an email invitation to connect to Onpatient, our EHR-integrated secure patient portal.  Please accept the email invite, create a password, and log in to your account to activate it within 24 hours of receiving the email invite, as the invitation link will expire after 24 hours.  If you cannot find the email or need the link resent, please contact us.

  • New Client Registration: creating an Onpatient account is required in order to complete new client forms and confirm your first appointment. Please see the following attached document for further instructions:

 

Location

 

The office is located on the 4th floor of the StudioWorks Ballard Building, at 1417 NW 54th St, Seattle, WA 98107.

The building does not have reserved parking. There is limited 2-hour street parking directly in front of the building on NW 54th St, and additional free street parking around the corner on 14th Ave NW.

  

To access the building, use the callbox to the right of the front door. Dial 460 or use the alphabetical name directory to call Brooke Heyman to be buzzed in. Please always use the callbox, even if the door is open, as it lets me know you have arrived.  Take the elevator to the 4th floor and have a seat in the waiting area next to the elevator and I will greet you there.
